I understand that mass communication equipment and facilities are available to me only for assigned mass
communication class projects and approved activities. I may not check out equipment or employ facilities
for personal use of any kind.
I will not bring food or beverages into ANY of the production facilities.
I understand that failure to comply with food, beverage, and smoking restrictions will result in the loss
of any/all scheduled production time for one week.
I understand that failure to return any equipment which has been checked out to me (1) in good and working
order and (2) at the scheduled return time will result in the immediate loss of check-out privileges for at
least one 24 hour period or cancellation of all check-out privileges until restitution is made or arranged
for damaged equipment.
I agree to pay for loss or damage to equipment or facilities. I understand that my instructors may impose
additional sanctions for (1) damage or abuse that is the result of negligence or (2) the late return of
equipment. Instructors will be notified about late or damaged equipment returns.
I agree to notify the appropriate staff person(s) immediately in case of problems or damage to equipment.
I understand that I may NOT take any mass communication equipment into a situation that may put that
equipment at risk (e.g., crowded bars, parties, skydiving, etc.)
I understand that I am subject to University Procedures concerning student conduct regulations (see Part
I, Prohibited Actions and Improper Conduct, of the Student Handbook, 1993-1994). Part I covers academic
dishonesty and the attempted malicious defacement of academic resources including mass communication
computer, audio, and video equipment. I understand that malicious mishandling of mass communication's
computer, audio, and/or video equipment may result in severe consequences including possible course
failure and/or suspension from the University (see Part III, Disciplinary Procedures).
